2-Week Feedback Form
At this point, you’ve completed about two cycles of your plan. The first week was likely about getting a feel for the structure and learning new exercises. By the second week, you should have some familiarity with your program.
In this form, I’m asking for two kinds of feedback to improve your experience (and that of future clients’): technical feedback and programming feedback. Technical feedback helps me identify and reduce any obstacles that make the program harder to follow. Programming feedback is your chance to tell me what is and isn’t working in your program so that I can update it for the next two week cycle.
Please answer thoughtfully. If you give a rating that could use explanation, include details in the follow-up text block. The more specific you are, the better I can optimize your next plan and improve the overall experience.
